Overdose deaths involving prescription opioids killed more than 17,000 Americans in 2017, marking a five-fold increase since 1999. High prescribing rates of opioid analgesics have been a substantial contributor to prescription opioid misuse, dependence, overdose and heroin use. There was recognition approximately ten years ago that opioid prescribing patterns were contributing to this startling increase in negative opioid-related outcomes, and federal actions, including Medicare reimbursement reform and regulatory actions, were initiated to restrict opioid prescribing. The current manuscript is a description (...) of those actions, the effect of those actions on opioid prescribing and related patient outcomes. We also describe our proposal of methods of expanding these efforts as an important piece to further reduce opioid-related misuse, dependence, and overdose death. (shrink)

We present Woodin's proof that if there exists a measurable Woodin cardinal δ, then there is a forcing extension satisfying all $\Sigma _{2}^{2}$ sentences ϕ such that CH + ϕ holds in a forcing extension of V by a partial order in V δ . Previous work suggests that threat-related stimuli are stored to a greater degree in working memory compared to neutral stimuli. However, most of this research has focused on stimuli with physically salient threat attributes, failing to account for how a “neutral” stimulus that has acquired threat-related associations through differential aversive conditioning influences working memory. The current study examined how differentially conditioned safe and threat stimuli are stored in working memory relative to a novel, non-associated stimuli. Participants completed a differential fear conditioning (...) task followed by a change detection task consisting of three conditions across two loads. Results revealed individuals successfully learned to distinguishing CS+ from CS– conditions during the differential aversive conditioning task. Our working memory outcomes indicated successful load manipulation effects, but no statistically significant differences in accuracy, response time, or Pashler’s K measures of working memory capacity between CS+, CS–, or N conditions. However, we observed significantly reduced RT difference scores for the CS+ compared to CS– condition, indicating greater RT differences between the CS+ and N condition vs. the CS– and N condition. These findings suggest that differentially conditioned stimuli have little impact on behavioral outcomes of working memory compared to novel stimuli that had not been associated with previous safe of aversive outcomes, at least in healthy populations. The way language as a human faculty has evolved is a question that preoccupies researchers from a wide spread of disciplines. In this book, a team of writers has been brought together to examine the evolution of language from a variety of such standpoints, including language's genetic basis, the anthropological context of its appearance, its formal structure, its relation to systems of cognition and thought, as well as its possible evolutionary antecedents. The book includes Hauser, Chomsky, and Fitch's seminal and (...) provocative essay on the subject, 'The Faculty of Language,' and charts the progress of research in this active and highly controversial field since its publication in 2002. The presence of temporal adjectives in possessive nominals like John's former car creates two interpretations. On one reading, the temporal adjective modifies the common noun (N-modifying reading). On the other, it modifies the possession relation (POSS-modifying reading). An explanation for this behavior is offered that appeals to what occurs in possessive sentences like John has a former car (N-modifying reading) and John formerly had a car (POSS-modifying reading). In the sentential cases, the source of two readings is two distinct, modifiable (...) phrases. Given the parallels, we propose a structure for possessive nominals analogous to that of possessive clauses. Specifically, we argue that such nominals include a locative small-clause structure, following Freeze (1992), and we explain the ambiguity structurally, as a simple matter of where temporal adjectives attach (NP vs. PP). We show that this analysis provides a straightforward basis for the semantic composition of possessive nominals. (shrink)

In 1914, James Leuba, a psychologist at Bryn Mawr, conducted several surveys of scientists and college students regarding their religious beliefs, publishing his findings in a 1916 book titled The Belief in God and Immortality. Among scientists generally, 41.8 percent indicated they were believers in a personal God (defined as a being to whom one could pray, expecting a response), whereas 41.5 percent expressed disbelief in such a God and 16.7 percent declared themselves to be agnostic. Among elite scientists (those (...) with an asterisk by their names in James McKean Cattell's American Men of Science), the percentage of believers was lower, at 31.6 percent. Among elite biologists, the subset who believed in God was even smaller—16.9 percent. In 1996 and 1998, Edward Larson and Larry Witham replicated Leuba's study, publishing their findings in the April 23, 1997, and July 23, 1998, issues of Nature. Their surveys revealed that of all scientists questioned, 39.3 percent professed belief in a personal God, about the same as in the 1914 study. However, among elite scientists—now defined as members of the National Academy of Sciences—the proportion who were believers had plummeted to 7 percent, with biologists showing the least religious conviction at 5.5 percent. In the general population of the United States, some 86 percent profess belief in the existence of a personal God, according to a 1999 Gallup poll. These figures dramatically indicate the great no-mans land separating the religious convictions of ordinary citizens from those of the scientific community, especially its leading members. This dissensus has fueled many of the bitter battles recently fought over evolution and stem cells and has ignited explosive devices laid along several political byways. (shrink)

Conflicts of interest have been reduced to financial conflicts. The National Institutes of Health’s new rules for managing conflicts of interest in medical research, the first major change to the regulations in over 15 years, address only financial ties. Although several commentators urged that the regulations also cover non-financial interests, the Department of Health and Human Services declined to do so. Similarly, the Institute of Medicine’s influential 2009 Conflict of Interest Report focuses almost exclusively on financial conflicts. Institutional policies at (...) academic medical centers and guidance from professional bodies and medical journals also primarily emphasize financial ties. Even broadly worded rules are applied more readily to financial ties than non-financial interests, such as the regulations that restrict institutional review board members with conflicting interests from participating in protocol reviews. Taking Isaac Newton at his own word, historians have long agreed that the decade of the 1660s, when Newton was a young man in his twenties, was the critical period in his scientific career. In the years 1665 and 1666, he has told us, he hit on the ideas of cosmic gravitation, the composition of white light, and the fluxional calculus. The elaboration of these basic ideas constituted his scientific achievement. Nevertheless, the decade of the 1660s has remained a virtual (...) blank in our knowledge of Newton. It need not remain so always. His papers contain a wealth of manuscripts from his undergraduate years and the period immediately following. The first volume of his mathematical papers, which will soon be published, will demonstrate how extensive the information on his early mathematical development is. The development of his non-mathematical studies, especially of what I shall call his scientific studies to distinguish them from the mathematical, can be followed as well—in his reading notes, in his notebooks, above all in the passage in his philosophical notebook labelled Quaestiones quaedam Philosophicae. In this passage we see emerging into consciousness for the first time the questions on which Newton's philosophy of nature was built. (shrink)

The human science or qualitative approaches to research have always argued that methodology must be determined by the subject matter under study. Yet the same approaches to data collection (i.e., the qualitative interview) and data analysis have been utilized by these approaches since their inception. The most essential lesson of van den Berg's metabletics is that no phenomenon is static or absolute. If human phenomena are ever-changing then the methodologies we use to study them must also change and adapt, so (...) that we can more fully and authentically capture their meaning structures. This paper will develop this argument, and demonstrate the limitation of interviews for the study of the changing nature of human phenomena, utilizing psychotherapy research as an example. (shrink)

How transcription factors achieve their in vivo specificities is a fundamental question in biology. For the Homeotic Complex (HOM/Hox) family of homeoproteins, specificity in vivo is likely to be in part determined by subtle differences in the DNA binding properties inherent in these proteins. Some of these differences in DNA binding are due to sequence differences in the N‐terminal arms of HOM/Hox homeodomains. Evidence also exists to suggest that cofactors can modify HOM/Hox function by cooperative DNA binding interactions. The Drosophila (...) homeoprotein extradenticle (exd) is likely to be one such cofactor. In HOM/Hox proteins, both the conserved ‘YPWM’ peptide motif and the homeodomain are important for interacting with exd. Although exd provides part of the answer as to how specificity is achieved, there may be additional cofactors and mechanisms that have yet to be identified. (shrink)

Although researchers have begun to examine how firms manage their entire web of stakeholder relationships, the component relationships also require theoretical and empirical examination. Several studies have found that Employee Stock Ownership Plans (ESOPs) have a positive impact on firm performance. The authors explain these results by hypothesizing that ESOPs, when combined with employee participation programs, forge a stakeholder relationship between management and employees. The authors offer criteria for identifying stakeholder relationships, provide background on ESOPs, analyze why they contribute to (...) establishing such a relationship, and examine how published empirical research supports this analysis.
